While Pathway Plans are typically led and completed by Local Authorities, those working directly with young people play a crucial role in bringing these plans to life through everyday practice, key working, support planning, and independence-focused interventions.

Why This Course Matters

A strong Pathway Plan can make the difference between a young person feeling supported and empowered, or overwhelmed and unprepared. While responsibility for producing Pathway Plans usually sits with the Local Authority, providers and frontline practitioners are often responsible for supporting the young person to achieve the goals and outcomes identified within them. This course will help participants:

✔️ Understand the purpose, structure, and legal expectations of Pathway Plans
✔️ Translate Pathway Plan objectives into meaningful day-to-day support
✔️ Engage young people meaningfully in working towards their goals and aspirations
✔️ Link Pathway Plans to support plans, risk assessments, key working, and independence programmes
✔️ Use Pathway Plans to evidence progress, outcomes, and successful transitions towards adulthood

Course Highlights

  • Understanding Pathway Planning – Legal framework, purpose, responsibilities, and the provider’s role
  • Turning Plans into Practice – Using Pathway Plans to inform support, interventions, and key working sessions
  • Person-Centred Support – Helping young people identify and achieve their goals, aspirations, and next steps
  • Linking Plans to Outcomes – Education, employment, health, relationships, accommodation, and independence
  • Evidencing Progress & Preparing for Adulthood – Recording achievements, measuring outcomes, and supporting successful transitions

Who Should Attend?

This course is ideal for key workers, support workers, personal advisors, social workers, managers, and other professionals involved in supporting care leavers and young people preparing for independence. It is particularly relevant for providers who use Pathway Plans to shape support, monitor progress, and demonstrate how they are helping young people move successfully towards adulthood.
