Pathway Plans are a key tool in preparing young people for adulthood, yet they can often become process-driven rather than purpose-driven. This session will help you reframe planning as a dynamic, relational process that reflects each young person’s journey, aspirations, and evolving needs.

Why This Course Matters

A strong Pathway Plan can make the difference between a young person feeling supported and empowered, or overwhelmed and unprepared. This course will help participants:
✔️ Understand the purpose, structure, and legal expectations of Pathway Plans
✔️ Develop individualised, realistic, and outcome-focused plans
✔️ Engage young people meaningfully in the planning process
✔️ Link Pathway Plans to risk assessments, care/support plans, and independence skills
✔️ Use Pathway Plans as a tool to evidence progress and support transitions

Course Highlights

🔹 Understanding Pathway Planning – Legal framework, purpose, and responsibilities
🔹 Person-Centred Planning – Capturing voice, identity, and aspirations
🔹 From Paper to Practice – Making plans active, relevant, and regularly reviewed
🔹 Linking Plans to Outcomes – Education, employment, health, relationships, and independence
🔹 Common Pitfalls & Good Practice – What works and what to avoid

Who Should Attend?

This course is ideal for key workers, personal advisors, social workers, managers, and support staff involved in developing or reviewing Pathway Plans. It is particularly relevant for those working with care leavers and young people preparing for independence.
